Please let us know who has permission to pick up your child/ren from Hebrew School or if they have permission to self-dismiss. (Please also indicate if there is someone who does not have permission to pick up your child.)

We welcome all learners. If you answered yes, we would like to speak with you (and your student if applicable) before the beginning of the school year, to discuss the ways we can best collaborate with your family to give the best support in the school. If you are comfortable, we invite you to send any documentation you have to families@tandv.org.  
If your child takes medication, please include the dosage & timing. While T&V will not be dispensing any medication, it is important that we know in case of an emergency.

By submitting this form, my child/ren has/have permission to participate in Kulanu @ T&V. In consideration of my child/ren's acceptance as a Hebrew School student, I hereby waive any and all claims against Town & Village Synagogue, its agents and its employees that may arise out of any injury, loss or damage suffered by my child/ren during any Kulanu activity. I hereby authorize the Youth and Family Facilitators, or person designated by the Youth and Family Facilitator, to obtain emergency medical care for my child/ren in the event such care is indicated. I give my permission for my child/ren to receive emergency medical care by any nurse, doctor, paramedic or member of a medical staff of a hospital licensed by the State of New York. I understand that every effort will be made to notify a parent/guardian prior to treatment.

I certify that my child/ren is/are in good physical health. They have my permission to participate in all activities that are part of the regular Kulanu program. Field trips may be arranged by Town & Village Synagogue, and transportation may include bus or vehicle driven by a classmate's parent or guardian.

I hereby consent to the participation in interviews, the use of quotes, and the taking of photographs, movies or videos of my child/ren. I also grant the right to edit, use, and reuse said products for nonprofit purposes including use in print, on the internet, and all other forms of media. I also hereby release Town & Village Synagogue and its agents and employees from all claims, demands, and liabilities whatsoever in connection with the above.

Town & Village Synagogue will adhere to the following limitations on the rights granted to it:
- The names of children under the age of 18 will not be published at any time, except with the parent's prior permission.
- Requests by adults that photographs of themselves or their minor children be removed from the T&V website or other public online sites will be honored.

Congregation, Community, & Commitment
At Town & Village Synagogue, we are committed to lifelong Jewish learning. 
Our goal is to provide all students this foundation in childhood. In order to become B'nei Mitzvah at T&V, we expect that students will participate in a meaningful Jewish educational program, and we encourage Shabbat service attendance. By submitting this form, you are agreeing to this commitment toward education.

The work of raising and educating Jewish children cannot be done in the school alone, but is a project of the school, the family and the community. When you join T&V's Youth and Family community, and spend time learning and volunteering with us, you make a powerful statement to your children that Jewish learning is a lifelong process, and that we are all on the same team in creating these experiences for them.

Families are asked to attend occasional special programs with their children, which helps to build the home-synagogue connection.

We are also in need of regular volunteers at Kulanu. If you would consider sharing your time and talents with our school, please indicate below where your skills might best be put to use.
(i.e. family programs, social action/tikkun olam, Shabbat programs, class parent, family engagement, etc.)

EARLY BIRD DISCOUNT: Register before August 10th and receive a $200 discount per child
MULTI-CHILD DISCOUNT: Families with more than one child enrolled will receive a $200 discount per family.
